New AARP Report: Unpaid Care Skyrockets to over $1 Trillion

AARP’s latest report reveals a staggering surge in the economic value of unpaid family caregiving, now exceeding $1 trillion. Since 2006, AARP’s Public Policy Institute has tracked this value through its Valuing the Invaluable reports. Last week, at a virtual media briefing, the organization released its seventh report, Valuing the Invaluable 2026: Family Caregivers’ Contribution Reaches $1 Trillion,  delivering a strong message: in 2024, family caregivers provided nearly 50 billion hours of mostly uncompensated care—valued at a whopping $1.01 trillion. Sixteen years earlier, AARP estimated the value of uncompensated care was just $350 billion.

 Building on the previous data, the 9-page report, released on March 26, 2026, by AARP’s Public Policy Institute, emphasizes throughout that caregiving impacts not just families, but also the economy, labor market, and healthcare system.

 It estimates that 59 million caregivers contributed a total of 49.5 billion hours of care each year, valued at $20.41 per hour. Using the Caregiving in the US 2025 data set (which uses new statistical methods that incorporate the range of tasks and support that family caregivers provide), this amounts to about $1.01 trillion annuallyFor comparison, family caregivers’ economic contribution surpasses the combined federal, state, and local Medicare spending in 2024 ($931.7 billion) and nearly doubles out-of-pocket health care spending ($556.6 billion).

 To further accentuate the magnitude, the report calculates that the nearly 49.5 billion hours of uncompensated care provided annually equal the labor of about 24 million full-time workers—that’s 17% of the nation’s full-time workforce.

According to the AARP report, American adults spend as much time on caregiving duties as on everyday activities, such as housecleaning and preparing meals, and almost twice as much time as they spend on religious activities and volunteering combined.

This unpaid care—delivered by caregivers to older adults, neighbors, and friends—enables millions to remain independent in their homes and communities. The report warns that, without this assistance, millions of Americans would be forced to rely on assisted living or costly nursing facility care, resulting in significantly higher costs for public programs such as Medicaid. However, it also recognizes that this support comes at a steep personal cost to caregivers, often undermining their health, emotional state, financial security, and general well-being.

 Beyond national implications, AARP’s caregivers report also provides state-by-state estimates of the number of caregivers, total hours provided, economic value, and the average hourly value of care.

These state figures, for example, show that the estimated value of caregiving ranges from $14.12 per hour in Louisiana to $27.05 per hour in Washington, reflecting regional differences. In Rhode Island, approximately 155,000 family caregivers provide 111 million hours of unpaid care, valued at $2.8 billion, or about $25.07 per hour.

Continuing this deeper look, the AARP report finds that family caregivers now provide more care than ever, averaging 27 hours per week. More than half—57%—deliver high-intensity care: tasks such as bathing, dressing, wound care, and administering injections.

 Presser Announces New Caregiver Report’s Release

 To explore the report’s findings in greater depth, AARP convened a 40-minute panel moderated by Ilse Zuniga, Director of External Relations. This discussion brought together Dr. Myesha Minter-Jordan (AARP CEO), Nancy LeaMond (Executive Vice President and Chief Engagement Officer), Paula Cunningham (AARP Michigan State Director), Rita B. Choula (Senior Director at AARP’s Public Policy Institute and lead author), and Megan O’Reilly (Government Affairs Department).

During the panel, Dr. Myechia Minter-Jordan, AARP’s CEO and physician, addressed the 30 journalists present, stating that family caregivers are underwriting a service that millions depend on daily.

 She emphasized that, with family caregiving now exceeding $1 trillion annually, employers, healthcare providers, and policymakers must do more to recognize and support caregivers as they fill essential gaps in the healthcare system.

Dr. Jordan said caregiver stories are powerful and often painful, stressing that AARP is working to magnify these voices and advocate for concrete policies that recognize caregivers’ economic contributions.

 “Given that most Americans will either be a caregiver or need one at some point in their lives, we need to do better,” Dr. Jordan told the journalists.

 “AARP is supporting bold solutions to assist America’s caregivers, says Dr. Jordan. “These include a national paid family and medical leave policy, as well as greater respite services and resources, such as those offered in states through partnerships with United Way and AARP’s 211 Caregiver Support Hotline.”

She concluded that the released report should be a wake-up call and an opportunity for action, hoping that it will drive real, long-lasting change.

Nancy LeaMond, AARP Executive Vice President and Chief Advocacy and Engagement Officer, noted, “Caregiving is not simply a family issue; it’s a labor force, economic, and healthcare issue that compels action. Behind every data point is a person—a daughter, husband, grandchild, or neighbor.”  

According to LeaMond, AARP has been working relentlessly to raise caregiving as a national priority and to push for common-sense, bipartisan solutions that can save caregivers time and money.

LeaMond added that the organization has worked to raise the visibility of caregiving, making it a national priority at both the state and federal levels. She noted, “Oklahoma became the first state in 2023 to pass a comprehensive state-wide caregiver tax credit to put money back in the pockets of family caregivers,” with Nebraska following the next year.

Additionally, she stated that 12 other states have considered enacting statewide caregiver tax credit legislation during the 2026 legislative session.

LeaMond further reported that on Capitol Hill, AARP has been advancing legislation such as the Credit for Caring Act, which offers a $5,000 federal tax credit to offset caregiving expenses, and the Lowering Costs for Caregivers Act, which allows family caregivers to use Health Savings Accounts or Flexible Savings Accounts for care expenses for parents.

Paula Cunningham added a personal dimension, relating stories to illustrate the intense, unceasing nature of caregiving. She narrated stories of caregivers forced to draw from their savings or reduce work commitments, such as Deb Conja, an attorney from Okemos, Michigan, who left her job to care for her mother.

She also shared the experience of a Detroit military veteran who, after two tours of service, described caregiving for her mother as the hardest job she’s ever had—an indication of the intensity of these responsibilities.

“Navigating through who to call and when to call, and what kind of services are available is another layer of stress that we’re trying to help reduce,” says Cunningham. She noted that AARP Michigan is calling for funding for a Caregiving Resource Center to provide one-stop online shopping and connect 1.6 million Michigan caregivers across the state with the support services they need.  

 AARP Michigan is also pushing to expand access to funding for home- and community-based services that are necessary for caregivers to stay in the workplace, says Cunningham.

 Finally, during the Q&A, Megan O’Reilly from AARP’s government affairs department responded to Politico journalist Robert King about whether a federal crackdown on fraud in personal care services could undermine governmental efforts to support family caregivers. “Fraud is a crime, and those who commit fraud should be held accountable. But we have to make sure that we’re protecting the care and the need for the essential care and services that our communities and loved ones need to remain at home…,” she said.  

 In response to a question, Rita B. Choula, the AARP report’s lead author, provided extra insights as to how the economic value of care was calculated for the latest report.  

Today, caregiving encompasses much more than just taking someone to a provider’s office, paying bills, or even mowing the lawn, says Choula, noting that the economic value of care must account for the complexity of the care provided.  “Individuals are now doing things in the home that medical providers and professionals were trained to do,” she notes, explaining that these new duties have resulted in using a higher hourly wage to calculate the cost of unpaid care.  

AARP provides resources to help families navigate the myriad of caregiving challenges by connecting them to reliable resources in every state. AARP’s state-by-state Family Caregiver Resource Guides help family caregivers access key programs, services, and agencies right in their community. Additionally, through its partnership with United Way Worldwide, caregivers can access local support services in 28 states by calling 211, including help finding in-home care, respite care, transportation, and other essential services.

A Final Note…

Urgent policy action must be taken by Congress and state legislatures to provide the concrete financial support needed to deliver real financial relief and structural support for America’s caregivers through measures such as tax credits and paid family leave. It is the right thing to do!

Expanding Medicare on political agendas: In-home Health Care critically important

This week Vice President Kamala Harris unveiled a “Medicare at Home” proposal on ABC’s The View that would expands Medicare to assist older Americans to age in place at home by covering some of the cost of in-home care. The proposal targets adults who are part of the ‘sandwich generation,’ estimated to be 105 million Americans who are raising children along with taking care of their elderly parents.

The Medicare benefit to assist caregivers would propose to have cost-saving benefits for the federal government by allowing seniors to stay at home rather than being sent to costly nursing homes. It would also reduce hospitalizations, too.

Harris told about her personal experience as a caregiver, providing care to her mother, Shyamala Gopalan, a biomedical scientist, who died of cancer in 2009 at the age of 70. Caring for a parent can translate into “trying to cook what they want to eat, what they can eat,” she said. “It’s even trying to think of something funny to make them laugh or smile,” she added.

“We’re talking about declining skills” of older people, “but their dignity, their pride, has not declined,” Harris added.

“There are so many people in our country who are right in the middle. They’re taking care of their kids and they’re taking care of their aging parents, and it’s just almost impossible to do it all, especially if they work,” Harris said.  “…we’re finding that so many are having to leave their job, which means losing a source of income, not to mention the emotional stress,” she said, explaining why there is a need to expand Medicare to cover more in-home care services.

The Costs

The Brookings Institution recently estimated that a “very conservatively designed” program would cost $40 billion a year. They noted that “controlling demand in such a program is nearly impossible – for reference, Medicaid, which covers far fewer adults than Medicare, actually spent $207 billion on long-term services and supports in 2021”.

In addition, “Home health is such a hotbed of fraud,” said Theo Merkel, a health policy expert at the Paragon Health Institute and the Manhattan Institute. “If the proposal is adopted, taxpayers could end up paying for everyone who stays at home with their Medicare-eligible family member as a government paid Service Employees International Union member.”

The Cato Institute, a libertarian think tank headquartered in Washington, D.C., charges that Harris’ new Medicare home care benefit is “uncompassionate, fiscally reckless, and a corrupt attempt to buy the votes of Medicare enrollees and their middle-aged children in an election year.”

Examining the Differences…

According to Matthew E. Shepard, Communications Director for the Center for Medicare Advocacy, the new Harris proposal is quite different from the existing home care benefits that Medicare’s 65.5 million enrollees receive. ”The new proposal focuses on Long Term Services and Supports, something of a term of art in the health care world. While details are scarce, it would provide, we believe, ongoing affordable home care aide service without a need for skilled care or that strict definition of homebound,” said Shepard.  The proposal’s funding would come from increased savings in Medicare Part D as the list of negotiable drugs grows  [a historic provision of the Inflation Reduction Act which is lowering the cost of senior’s medication]  savings currently estimated at $6 billion in 2026, and which will only grow as more drugs are added, he noted.

“We are going to save Medicare that money, because we’re not going to be paying these high prices [for drugs] and that those resources are then put to use in a way that helps a family,” Harris said.

The Trump proposal

The Trump/Vance campaign quickly issued a statement taking credit for already making a commitment to America’s seniors receiving at-home care, saying that Harris’ Medicare expansion policy was just following his lead. Former President Trump released his home care platform last summer, according to an Oct. 8th statement. “Specifically, President Trump will prioritize home care benefits by shifting resources back to at-home senior care, overturning disincentives that lead to care worker shortages, and supporting paid family caregivers through tax credits and reduced red tape,” noted the statement.

One of Trump’s 20 point platforms is “Fight for and protect social security and Medicare with no cuts, including no changes to the retirement age”. In the accompanying 16-page document, which, supports Medicare it says, “President Trump has made absolutely clear that he will not cut one penny from Medicare or Social Security. American citizens work hard their whole lives, contributing to Social Security and Medicare. These programs are promises to our Seniors, ensuring they can live their golden years with dignity. Republicans will protect these vital programs and ensure Economic Stability. We will work with our great Seniors, in order to allow them to be active and healthy. We commit to safeguarding the future for our Seniors and all American families. We will strengthen Medicare. Republicans will protect Medicare’s finances from being financially crushed by the Democrat plan to add tens of millions of new illegal immigrants to the rolls of Medicare. We vow to strengthen Medicare for future generations.”

 Dementia caregiving already set to quadruple in 2025

AARP notes on their website that one expansion of caregiver coverage, “a program for dementia patients and their caregivers that launched this year will quadruple in 2025, serving more of the country. The program, called Guiding an Improved Dementia Experience (GUIDE), provides a 24/7 support line, a care navigator to find medical services and community-based assistance, caregiver training and up to $2,500 a year for at-home, overnight or adult day care respite services. Patients and their caregivers typically won’t have copayments”.

Praise for expanding Medicare benefits

“We have long championed the expansion of federal support for long-term care,“ says Max Richtman, President and CEO, National Committee to Preserve Social Security and Medicare (NCPSSM), noting that Harris’ proposal gives that cause an enormous boost.

“Expanded Medicare coverage for home health care also would provide relief to millions of ‘sandwich generation’ Americans, who are struggling to provide care for their elderly relatives while also raising children.  Those ‘sandwich generation’ members are not Medicare beneficiaries, but would most definitely benefit from Harris’ long-term care plan,” says Richtman in an Oct. 8 statement.

According to Richtman, the plan also would add hearing and vision coverage to traditional Medicare. “Proper hearing and vision care are essential to healthy aging — but too many beneficiaries forgo it due to cost and lack of coverage. It is long past time that those coverages be added,” he added.  

Co-Director David Lipschutz says that the Center for Medicare Advocacy (CMA) strongly supports the proposed enhancement of Medicare coverage for on-going home care. “Access to services and supports in the home for those who are unable to independently perform activities of daily living would provide immeasurable help to millions of beneficiaries and their families and is an important step forward for the Medicare program,” says CMA’s Lipschutz. To maximize access to care for people who need it, expansion of home care coverage in Medicare should be combined with enforcing the benefit that exists now, he suggests. 

“Recognizing that most older persons and those with disabilities prefer to remain at home when they need help with daily living tasks, the Senior Agenda Coalition has worked for years to increase access to home and community-based care at the state level as these services are one of the biggest gaps in Medicare,” says Maureen Maigret, Policy Advisor for Senior Agenda Coalition of RI.  To include them in Medicare will lift a financial burden on both recipients and family caregivers as home care costing at least $35/hour that  can be out of reach for far too many who need these services to stay at home,” she says.

“We have not seen many details about the plan, but it would be important to make sure that Medicare provider reimbursement levels are sufficient to allow direct care staff to earn livable wages in order to have workforce sufficient to meet the demand,” note Maigret. “This new Medicare home care benefit should also be a boon for states as it can prevent persons from spending down their resources to a level where they become eligible for state Medicaid and need costly nursing home care,” she says.  

In a new paper for O’Neill Institute for Georgetown LawMcCourt Professor Judith Feder and Nicole Jorwic explore how adding a home care benefit can help beneficiaries and family caregivers. “While this new benefit would not reach the full population in need of long-term care, paired with investments in Medicaid, it’s a good strong start-and given our nation’s resources, clearly within our means,” say the authors. 

“A support system that relies on unpaid family members and underpaid workers is simply not sustainable for the future,” warn the authors.

“Our failure to make Medicare “whole” by addressing Long Term Services and Support needs is not about a shortage of resources, it’s about a shortage of political will. It’s time the nation stepped up,” they say.

Pay attention to Caregiver voters

AARP is nonpartisan and does not take a position on campaign proposals, though AARP has previously said financial relief is needed to help individuals age in place at home and support family caregivers, says Sarah Lovenheim, AARP’s vice president, external relations.

According to AARP’s “She’s the Difference” survey released last month, 96% of woman aged 50 and over say they are highly motivated to vote in the upcoming elections, making them one of the most driven and key voting groups.

“Any political candidate would be wise to pay attention to the concerns and needs of caregivers today. Voters over age 50, who disproportionately make up America’s 48 million plus caregivers, could make or break elections up and down the ballots,” says Nancy LeaMond, AARP’s executive vice president and chief advocacy and engagement officer. “From recent battleground polls, we know that roughly one-third of swing voters over age 50 identify as family caregivers,” she notes.

“Supporting family caregivers is an urgent need – not only for families struggling to get by but for our country’s future,” warns LeaMond.

Regardless of who wins the election, a Medicare at home proposal cannot happen without Congressional support. As the presidential campaign winds down, older voters must make it extremely clear to lawmakers seeking their vote how they feel about expanding Medicare benefits.  

AARP report: States’ care for seniors “painfully inadquate”. Major gaps in RI

Last week, the Washington, DC-based AARP released its latest Long-Term Services and Supports (LTSS) Scorecard, calling care provided to seniors and persons with disabilities during the last three years after the COVID-19 pandemic “painfully inadequate.”  This nine-page report detailed state specific data, finds major gaps in providing care in every state, including Rhode Island, especially in these categories:  “Housing for Older Adults”, “Home Care Costs” and “Home Health Aide Supply”. 

The LTSS Scorecard, consisting of 50 indicators, focused on: affordability and access; choice of setting and provider; safety and quality; support for family caregivers; and community integration, using data from a variety of publicly available sources, such as the Centers for Medicare and Medicaid Services, American Community Survey, and Bureau of Labor Statistics.

Many benefits of accessing the data

According to AARP, this Scorecard can be a roadmap for improving the LTSS systems and the lives of those who rely on assistance.  Many others will benefit by reviewing this data, too. Federal, state, and local policy makers can identify priorities and ensure resources are allocated equitably. Aging advocates can glean public data and hold government officials accountable. Family caregivers can identify needed resources and become actively involved in policy discussions and advocacy. Seniors relying on long-term care programs and services can learn to inspire culture change and demand available tools. Finally, employers can improve workplace policies and practices to assist caregivers and healthcare groups so they can explore public-private partnerships and alliances. 

Like the findings reported in AARP’s LTSS Scorecard in 2020, Rhode Island ranked 12th in the country in 2023, but this year’s Scorecard reveals that the state has made some progress in improving care options for older Rhode Islanders, specifically the enrollment in the Program of All-Inclusive Care for the Elderly (PACE), as well as the availability of Adult Day Services.

“The pandemic reinforced the need to strengthen long-term care for countless loved ones across the country, including here in Rhode Island,” said AARP Rhode Island State Director Catherine Taylor in a Sept. 28th statement announcing the release of the 2023 LTSS Scorecard. 

“AARP’s Scorecard shows that there are many roads to meet the needs of all Rhode Islanders who deserve the very best care, including the 121,000 family caregivers in our state. The overwhelming majority — 87% — of Rhode Islanders age 45+ would prefer to receive care for themselves or loved ones at home with caregiver assistance. Yet, our low rankings in Housing for Older Adults (51st), Home Care Costs (43rd) and Home Health Aide Supply (41st) underscore how difficult it is for them to age the way they choose. It’s time to accelerate our efforts, for the sake of saving more lives,” she says.  

RI is dead last in housing

“In particular, Rhode Island’s dismal ranking for Housing for Older Adults – dead last – points to the urgent need to increase the supply of affordable, accessible housing. We can do that by passing legislation early in 2024 to strengthen homeowners’ ability to build an accessory dwelling unit (ADU) to allow them to age in place, support a family member, or house a caregiver,” Taylor said.

A national snapshot of the LTSS Scorecard’s 2023 Data

According to Scorecard, 12 states, including Rhode Island, have enacted paid family leave laws and 18 states have paid sick day laws, which can be used to assist caregivers. AARP’s latest Scorecard also reported that only 6 states provide a tax credit for family caregivers’ out-of-pocket expenses. Rhode Island does not provide a tax credit – nor is it among the 7 states that have statewide laws protecting caregivers from discrimination in the workplace that ensure they are not unfairly treated due to caregiving duties outside of work.

Meanwhile, it was reported that dozens of states experienced declines in the number of care choices that help support family caregivers. For instance, 16 states, including Rhode Island, had declines of 10% or more in access to home health aides since 2019. 

Regarding Home and Community-Based Services (HCBS), the LTSS Scorecard reported that 11 states, including Rhode Island, had state policies that improve presumptive eligibility for Medicaid HCBS at the time of data collection, making it possible for people to go home to receive care after being in the hospital – rather than having to be admitted to a nursing home while their eligibility for Medicaid payments is being determined.

The research indicated that many states also have large numbers of people with low care needs living in nursing homes, indicating a lack of HCBS access and services. More than 20% of nursing home residents in Rhode Island have low needs – compared to 9% nationally.

Data confirms Major Workforce Crisis in nursing facilities across US

As to nursing homes and institutional care, the AARP’s 2023 Scorecard confirmed that a  major workforce crisis exists in facilities across the country. Across all states, wages for direct care workers are lower than wages for comparable occupations, with shortfalls ranging from $1.56 to $5.03 per hour. In Rhode Island, wages are $1.79 lower than other entry level jobs.   

Nationally, more than 50% of nursing staff in nursing facilities leave their job within a year (53.9% turnover rate). However, in Rhode Island, the rate is slightly better than the average, at 49.3% – with Montana, Vermont, and New Mexico experiencing the highest averages in staffing turnover.  

Staffing disparities are also a significant challenge, say researchers, noting that residents of nursing facilities with high admissions of Black residents receive almost 200 fewer hours of care per year compared to residents of facilities with high admissions of white residents. 

According to the LTSS Scorecard, only 22% of nursing facility residents live in a facility with a 5-star rating in the US – with less than the national average, about 20.2%, of Rhode Island residents living in a 5-star facility. Gaps in workforce and equity result in persistent problems in care, say the researchers. One measure, pressure sores, can be life-threatening as they can lead to bone or joint infections, cancer, and sepsis. In this measure, 10% of facility residents nationwide experienced a pressure sore.

Finally, this year’s Scorecard announced that there is progress being made to create innovative and effective alternatives to traditional nursing facility models. Specifically, the researchers say that 10 states, including Rhode Island, made strides in nursing home innovations, such as by utilizing Green House® Nursing Home availability and policies, which includes small facilities, private rooms, and other best practices.

AARP’s 2023 LTSS Scorecard identified recommendations that can fix the nation’s delivery of long-term care programs and services to enable seniors to age in place in their communities. It called for increasing support to the nation’s 48 million family caregivers, “who are the backbone of the long-term care system, providing over $600 billion in unpaid care, such as with paid leave, tax credits, and other mechanisms to address health and financial needs.”

AARP Rhode Island continues to call on the Rhode Island General Assembly to support the state’s 121,000 family caregivers by advocating for enhancements to the state’s Temporary Caregivers Insurance program.

Recommendations on enhancing Long-Term Services and Supports

The Scorecard’s recommendations also called for investing in states’ Home and Community-based Care Infrastructure, by increasing support and training for home health aides and home visits, supporting the ability to access and use medical devices and equipment, and updating key Medicaid regulations and payment models.  

To tackle the workforce crisis, a recommendation suggests that it is crucial to support both nursing facility and in-home workforce by improving recruitment and training, increasing pay, and expanding the ability of trained nurses, aides, community health workers and other paraprofessionals to take on some aspects of care. States can choose to enact and enforce staffing and related care standards.

A recommendation also calls on States to expand the use of innovative, effective models for nursing homes can improve both quality of care and quality of life, such as with smaller facilities and private rooms and to address inequities by investing to close the staggering gaps in access to quality care and facilities and staffing shortages.

(Editor’s Note:  During the pandemic, then Governor Gina Raimondo allocated $5 million towards 2 changes in nursing homes – making all rooms private and providing non-shared, private bathrooms – for all residents – efforts to follow up on this effort have not resulted in what happened to that initiative according to the RI Department of Health).

Another recommendation urges states to create multisector plans for aging and building of coalitions and age-friendly health systems, to help seniors to age in place at home by having affordable and accessible housing and transportation, improved community design, and comprehensive emergency preparedness plans.

Researchers also suggested the advancing innovation in cities, counties, and states by supporting comprehensive state- and community-wide aging plans and piloting new approaches and programs, like Green House® Nursing Homes and presumptive eligibility, that can then be scaled.

Finally, Climate Change is increasing national disasters, and the LTSS Scorecard recommends that every state have a sound emergency preparedness plan to support nursing home residentsin particular, in times of crisis – including natural disaster.

“AARP’s LTSS Scorecard shows some progress and innovation, but there’s still a long way to go before we have systems that allow people to age well, and independently, for as long as possible and support the nation’s 48 million family caregivers. It’s also clear some emerging issues deserve more attention – from whether nursing homes are prepared to confront natural disasters, to whether they have plans in place to maintain and grow their workforces,” says Susan Reinhard, Senior Vice President, AARP Public Policy Institute.

The LTSS Scorecard is a charitable project made possible by a grant from AARP Foundation, with support from The SCAN FoundationThe Commonwealth Fund, and The John A. Hartford Foundation, and has been updated every three years since 2011.
