Republican Study Committee, Social Security and Medicare

Last month, the Republican Study Committee (RSC) unveiled its 167-page FY 2024 “Protecting America’s Economic Security” budget proposal which calls for balancing the federal budget in seven years, slashes $16.3 trillion in wasteful government spending over ten years, and cuts $5.1 trillion in taxes. During the 118th Congress (2023 to 2024), 175 House Republican lawmakers from 38 states are RSC members. With over 70 percent of the House Republicans belonging to the RSC, the release of the proposed budget is a dependable indicator of where the chamber’s caucus stands on key legislative priorities.

The RSC annually releases its own budget proposal for the next fiscal year during the time when both House and Senate Budget Committees prepare official budget resolutions. The fiscal blueprint provides the House Republican Caucus with an opportunity to detail its wish list of spending priorities and also provides its position on social issues.

RSC’s FY 2024 budget, released on June 14, 2023, is made up of 220 individual legislative proposals and initiatives received from its members. Reflecting GOP values, the proposed budget would eliminate most funding for new abortion policies, Critical Race Theory, and “gender politics” initiatives, while ensuring adequate funding for the military, continued construction of a Southern border wall; rolling back “climate change” programs, and eliminating increases in funding for the IRS. It would also make 2017 tax cuts permanent as its provisions are starting to expire soon, and ensuring the nation’s energy independence; providing tax benefits to promote R&D: restricting free meals for students, and the Supplemental Nutrition Assistance Program, with the possibility of work/volunteer requirements.  

Aging groups are concerned that the RSC budget also takes aim at Social Security and Medicare programs. During 2023, the annual Social Security trust fund report warned that the program will only be able to continue paying out full benefits through 2034. The Medicare trust fund also reported that it could run low on funds by 2028, two years later than reported last year. While the Democratic platform seeks to raise caps on some programs to address this, the RSC’s budget proposal floated additional policies to financially fix these programs.

The RSC budget proposes “modest changes” to benefits for those who are not near full retirement age. For future retirees, it calls for raising the full retirement age (taking into account for increases in life expectancy) from age 67 to age 69 for those who turn 63 in 2033. It would also reduce benefits for future beneficiaries who earned a “higher salary” before retirement. Also, only “modest adjustments” to the Social Security program as it operates would be made but it doesn’t clarify the changes.   

As to Medicare, the RSC budget calls for requiring disabled people to wait longer before they can receive Medicare benefits. It pushes for turning Medicare into a “premium support system,” where seniors would receive a subsidy to be used to purchase private health plans competing against traditional Medicare.

Point/Counter Point

With the release of RSC’s budget, House lawmakers, the White House, and aging groups quickly issued statements touting their own positions. 

RSC’s Chairman Kevin Hern (R-Ok) noted the GOP’s conservative values could be found on every page of the budget blueprint. “Our budget proves that fiscal responsibility is the only way to lower inflation, grow the economy, cut federal spending, empower taxpayers, and protect small businesses. Congress controls the purse strings, but we, the House, has failed to produce a budget year after year after year. Everyone has to balance their budget – governors, mayors, businesses, families – but not Congress. Nearly every problem facing our government can be traced back to our failure to both pass a budget and stick to it. The Republican Study Committee has a budget, and it balances in just seven years. Our budget is real, and it’s floor-ready. It’s time to get our country back on track,” he said.

“For too long, irresponsible spending habits in Washington have made the cost-of-living more expensive for hardworking American families, and they are fed up with business-as-usual. The RSC budget prioritizes smart, common-sense policy to empower the American worker, and it cuts back on wasteful spending to pay for what’s important – just like families have to do every day. We are committed to protecting our country’s economic security and restoring fiscal sanity to our Nation’s finances,” says RSC Budget and Spending Task Force Ben Cline (R-VA).

On the other hand, Pennsylvania Congressman Brendan F. Boyle, Ranking Member of the House Budget Committee, gives the RSC the thumbs down: “This budget stands in stark contrast to the positive, hopeful vision put forward by President Biden and supported by House Democrats: a government that works for working families, an economy where the ultra-rich pay their fair share, and a country where everyone has the freedom to retire with dignity. I look forward to working with President Biden and Congressional Democrats to ensure House Republicans’ bleak vision for America does not become our reality.”

Social Security and Medicare

Social Security advocacy groups warn that the programs proposed by the RSC would slash the nation’s safety net programs, like Social Security and Medicare.

“This budget would destroy Social Security as we know it. It would raise the retirement age and slash middle class benefits. These changes would transform Social Security from an earned insurance benefit, which replaces wages lost in old age, disability, or death, into a subsistence-level welfare benefit,” warns Nancy Altman, President of Social Security Works.

“The budget fearmongers about Social Security’s modest shortfall (still a decade away) rules out any options for raising revenue, such as requiring billionaires to contribute more. That leaves benefit cuts as the only “solution.” In other words, they want to cut benefits now to avoid cutting them later, which isn’t a solution at all. Indeed, the budget will increase the number of workers who will have no ability to retire while maintaining their standard of living,” says Altman.

“A particularly cruel provision would force disability beneficiaries to wait five long years (instead of the current two, which is already too long) before becoming eligible for Medicare benefits. Outrageously, this change would deprive some of the most medically vulnerable people in America of health care. This provision alone would inevitably lead to more medical bankruptcies and increased homelessness,” notes Altman. 

Correcting misinformation on Social Security and Medicare

The Washington, DC-based National Committee to Preserve Social Security and Medicare is launching a new public education campaign — sponsored by AARP — to correct misinformation about Social Security and emphasize the program’s value to American workers, especially to communities of color.  The campaign, “Social Security: Here Today, Here Tomorrow,” is intended to debunk myths and give workers the facts about their vital earned benefits.  The campaign includes a series of public town halls across the U.S. between June and October, 2023,  featuring prominent Social Security experts and advocates, Social Security officials, and financial advisors.

Max Richtman, President and CEO of the National Committee to Protected Social Security and Medicare says, “There is a ‘doom and gloom’ narrative about Social Security today. ‘The program is going bankrupt.’ ‘It won’t be there for future generations.’ ‘Politicians are stealing from Social Security.’ None of that is true. We want the public to understand that Social Security is there for them today — and it will be there for them tomorrow. And not just in retirement, but in case of disability, the death of a family breadwinner, or the retirement of a spouse. That’s what this campaign is all about.” Richtman will moderate some of the upcoming town halls.

“Social Security is a financial lifeline to millions of American seniors, but it is especially crucial to the Black community.  Black Americans traditionally rely on Social Security for monthly income more than other groups do, due to wage and job discrimination, diminishing employer-provided pensions, and challenges in saving for retirement. “Like all Americans, the Black community pays into Social Security with every paycheck — and deserves to know that the government will keep its promise to provide baseline financial security when they encounter what President Franklin Roosevelt called ‘the hazards and vicissitudes’ of life,” says Richtman.

House GOP leadership is committed to holding a vote to approve the RSC budget this year.  With a razor thin majority in the House and with Democrats stringing opposing, the RSC budget is unlikely to pass the lower chamber. 

In an interview with House Speaker Kevin McCarthy that aired Sunday, Jan. 29, 2023, on “Face the Nation,” McCarthy stated, “Medicare and Medicaid slashes are off the table.”  But with three fourths of the House GOP caucus endorsing the RSC budget, making cuts to Social Security and Medicare, older voters must make it clear to their House lawmaker, “Don’t touch Social Security and Medicare.”

A Call for Supporting the Nation’s Caregivers

Just 519 days until the 2024 Presidential Election (Nov. 5, 2024), older voters call on Congress to provide more support for the nation’s family caregivers, according to a new AARP Family Caregiving study, released on May 5, 2023, conducted by the bipartisan polling team of Fabrizio Ward and Impact Research.

Former First Lady, Rosalynn Carter, observed that “there are only four kinds of people in the world. Those who have been caregivers. Those who are currently caregivers. Those who will be caregivers, and those who will need a caregiver.” The results of AARP’s survey of U.S. registered voters, compiled in the 26-page report, U.S. Voters’ Views on Support for Family Caregivers, gives weight to the truth behind Carter’s quote as one-third of the voters in this study were caregivers in the past, one-fifth are currently caregivers, and half expect to be a caregiver in the future.

Caregivers Call for Government Support 

According to AARP, this is especially true for those age 50 and older: over two-thirds of voters, and 75% of voters 50+, say it is very important for Congress to help seniors live in their own homes. More than half (57%) say the same for supporting unpaid family caregivers, notes the poll’s findings.

An overwhelming majority of voters, 78%, are either a current, past, or future family caregiver. Over 70% of voters across the political spectrum say they would be more likely to support a candidate who backed proposals to support family caregivers, such as a tax credit, paid family leave, and more support and respite services.

“Family caregivers are the backbone of a broken long-term care system, providing $600 billion in unpaid labor each year and saving taxpayers billions,” said Nancy LeaMond, AARP Executive Vice President and Chief Advocacy and Engagement Officer in a statement announcing the release of the poll findings. “It is long past time for lawmakers to enact commonsense solutions that support family caregivers and help older Americans live independently in their homes, where they want to be,” she says.

The poll’s findings indicate that legislative caregiving proposals are strongly supported across party lines: 89% of Democrats and 72% of Republicans support expanding services to help seniors live at home in the community rather than in a  nursing home. Requiring minimum staffing standards in nursing homes so residents receive quality care is supported by 8 of Democrats and 74% of Republicans polled in this survey. 

The researchers say that black voters, women, Democrats, and Latinos are especially likely to see long-term care and support for caregivers as key issues to be addressed by Congress. Among those groups, 77% of women, 82% of Black voters, 71% of Latinos and 77% of Democrats say expanding services to help seniors live independently in their own homes instead of a nursing home is very or extremely important.

Most caregivers feel stressed emotionally, stretched thin in both time (18%) and money (18%), and overwhelmed; 40% of all family caregivers say they spend over 20 hours a week on caregiving, with almost half (48%) of women caregivers spending over 20 hours a week.

“Anxiety over caregiving is not limited to those currently providing care,” says AARP, noting that more than two-thirds of voters express concerns about whether they will be able to get the care they need as they grow older or live independently.

“The data is clear: regardless of your political stripes, people adamantly want to age at home rather than a nursing home,” said Tony Fabrizio, partner in Fabrizio Ward, noting “It’s why support for helping caregivers is so politically important.”

“Family caregivers are stressed emotionally and financially, and proposals to help them are overwhelmingly popular and key to helping older Americans remain in their homes,” said John Anzalone, founder of Impact Research. “This may be the sleeper issue of the 2024 elections with a voting group that makes up one in five Americans,” he says.

Caregivers Need Assistance

For many caregivers, providing care to their loved ones takes a big chunk out of their yearly finances, and time that they must give to providing the caregiving. According the AARP’s report, four in ten family caregivers report spending over 20 hours a week on their caregiving responsibilities, while roughly six in ten incur $1,000 or more in annual out-of-pocket expenses on items such as medical supplies and equipment, transportation, or home modification. Emotional stress, having to miss important events, and needing to balance their work, family, and caregiving responsibilities are impact the majority of family caregivers.

When asked what they would like Congress or their state government to do, family caregivers say they would like increased access to healthcare and financial support. Seven in ten voters ages 18 and older say they would be more likely to support a political candidate who backed legislative proposals to support family caregivers, such as paid family leave; a tax credit of up to $5,000; and expanded access to support and respite services. These policies would allow family caregivers an opportunity to take a break from caregiving, helping to prevent their burnout and allowing them to continue providing caring for their loved ones.

According to the polls findings, most voters ages 18 and older express concern about whether or not they will be able to get the care they need when they grow older. Roughly two-thirds says they are concerned they won’t have enough money to pay for the care they need, or they won’t be able to live independently at home, while six in ten are worried that they will need to live in a nursing home providing poor care.

The Support for Family Caregiving study was conducted by the bipartisan polling team of Fabrizio Ward and Impact Research for AARP. The poll used a sample of U.S. voters age 18 and older (with oversamples of voters age 50 and over, black voters, Hispanic voters, Asian and Pacific voters and family caregivers) using a combination of phone and text to web sampling. A total of 1,425 completed 20-minute in length interviews, were  conducted in English from April 4-10, 2023.

The margin of error for the 1,425 adults’ sample is +/- 3.4% at the 95% level of confidence. For the oversampled groups, the margins of error are: +/- 3.4% for voters 50+, +/- 6.9% for Black and Latino voters, +/- 8.9% for AAPI voters, and +/- 4.9% for voters 50+.

Warning: Being lonely and isolated is hazardous to your health

Last Sunday, just days before releasing an advisory or “public statement” raising the alarm about the devastating impact of loneliness and isolation in the United States, U.S. Surgeon General Vivek Murthy published an essay in the New York Times announcing its planned release. In his essay he also talking about his own personal struggles with loneliness and called for enhancing social connections to be made a “top public health priority”. 

One day before the release of Murthy’s new advisory on May 2nd, White House Press Secretary Karine Jean-Pierre said Murthy’s new advisory is a component of the Biden administration’s bigger efforts to address the nation’s mental health.  The releasing of this advisory was well-planned, being issued in May, designated as Mental Health Awareness Month in the US.

Sounding the alarm

The 81-page advisory report titled “Our Epidemic of Loneliness and Isolation,” finds that even before the COVID-19 pandemic, about half of U.S. adults reported experiencing measurable levels of loneliness.

While the advisory is “not an extensive review of the literature” the information was culled from electronic searches of research articles published in English and resources suggested by “subject experts,” with priority given to meta-analyses and systemic literature reviews.

As the nation’s chief advocate for public health, Murthy is using his office as a bully pulpit to issue an advisory calling for the nation’s immediate awareness and attention to the widespread epidemic of loneliness and isolation. He also provides a roadmap as to how it might be swiftly addressed.

According to the advisory, even before the onset of the COVID-19 pandemic, approximately half of U.S. adults reported experiencing measurable levels of loneliness. Disconnection fundamentally affects our mental, physical, and societal health. Loneliness and isolation increase the risk for individuals to develop mental health challenges in their lives, and lacking connection can increase the risk for premature death to levels comparable to smoking up to 15 cigarettes daily.

The advisory noted that the physical health consequences of poor or insufficient connection include a 29% increased risk of heart disease, a 32% increased risk of stroke, and a 50% increased risk of developing dementia for older adults. Additionally, lacking social connection increases risk of premature death by more than 60%.

While there are no new promises of federal dollars for tackling this societal problem, the Surgeon General’s advisory is tended to raise awareness and lays out a framework for a National Strategy to Advance Social Connection, which has never been implemented before in the United States. The advisory also provides suggestions about how specific groups – including governments, providers, researchers, health organizations, schools, high-tech companies, media, parents and caregivers, community-based and philanthropy organizations, workplaces and individuals – can take to increase connection in their lives, communities, and across the country and improve their health.

“Our epidemic of loneliness and isolation has been an underappreciated public health crisis that has harmed individual and societal health. Our relationships are a source of healing and well-being hiding in plain sight – one that can help us live healthier, more fulfilled, and more productive lives,” said Murthy, in a statement announcing the release of this advisory. “Given the significant health consequences of loneliness and isolation, we must prioritize building social connection the same way we have prioritized other critical public health issues such as tobacco, obesity, and substance use disorders. Together, we can build a country that’s healthier, more resilient, less lonely, and more connected.,” he said.

Brigham Young University Professor of Psychology and Neuroscience, Julianne Holt-Lunstad agrees with the Surgeon General’s assessment of the national importance of recognizing that loneliness and isolation can be hazardous to your health and to society’s wellbeing. “The advisory shows that too many Americans lack social connection in one or more ways and the evidence of the severe consequences have been growing for decades. While the pandemic helped raise awareness, since this was problem well before the pandemic, getting back to normal is not enough,” she says. 

“This advisory helps bring greater awareness to this important public health issue, and sets forth a framework for a national strategy –with detailed recommendations for various stakeholders—to begin to take action.  The personal and societal costs of inaction are far too high,” says Holt-Lunstad.

Along with affecting a person’s physical health, loneliness and isolation can contribute substantially to mental health challenges, too, says the advisory.  In adults, the risk of developing depression among people who report feeling lonely often is more than double that of people who rarely or never feel lonely.

Meanwhile, loneliness and social isolation in childhood increase the risk of depression and anxiety both immediately and well into the future. And with more than one in five adults and more than one in three young adults living with a mental illness in the U.S., addressing loneliness and isolation is crucial to fully addressing America’s mental health crisis, says the advisory. 

The advisory notes that while the epidemic of loneliness and isolation is widespread and has profound consequences for our individual and collective health and well-being of the nation, there is a simple way to attack this societal problem: social connection.

Social connection is beneficial for individual health and also improves the resilience of the nation’s communities. Evidence shows that increased connection can help reduce the risk of serious health conditions such as heart disease, stroke, dementia, and depression. Communities where residents are more connected with one another fare better on several measures of population health, community safety, community resilience when natural disasters strike, prosperity, and civic engagement.

Six pillars to advance social connection

This Surgeon General’s Advisory lays out a broad framework for the United States to establish a National Strategy to Advance Social Connection based on six pillars.

Pillar one calls for strengthening social infrastructure (parks, libraries, and playground) and the programs and policies in place.

The second pillar urges government at all levels to create “Pro-Connection” public policies like accessible public transportation or paid family leave to foster more connections in the family and community.  

Because loneliness and isolation are risk factors for patients with heart conditions, dementia and depression, the third pillar calls for utilizing heath care providers to assess and identify patients for risk of loneliness and intervene.

Pillar four calls for critically evaluating our relationship with technology to ensure that how we interact digitally with others doesn’t reduce meaning and healing personal connections with others.

More research is needed to study the impact of loneliness and isolation, beyond the evidence outlined in the advisory.  Pillar five calls for the deepening of our knowledge to understand the causes and consequences of social disconnection, populations at risk, and effectiveness of efforts to boost connections.

Finally, pillar six notes the importance of cultivating a culture of connection to influence the relationships of people we have in our daily lives.

Social engagement can help maintain good brain health 

For years, researchers involved in the publishing of 13 Global Council on Brain Health (GCBH) reports on various brain health topics including the impact of social isolation and loneliness. “We’re thrilled that the Surgeon General is focusing new attention and energy on this critical topic,” says  Sarah Lenz Lock, senior vice president of policy and brain health at AARP, also serving as GCBH’s executive director.

Both the GCBH reports and the Surgeon General’s advisory talk about the importance of social connections to health, says Lock. “The GCBH focused on how critical social engagement was to maintaining good brain health as you aged beginning in 2016, while the Surgeon General report in 2023 talked about all ages and health conditions, and followed the extreme social isolation brought about the pandemic that occurred afterwards,” she notes.  

According to Lock, the pandemic was a forced natural experiment that demonstrated the harm of loneliness and isolation which is what we had being saying long before.  But now we can see it in even more stark terms [after the pandemic],” she adds.

Lock sees the Surgeon General’s efforts to create a National Strategy to Advance Social Connections to be an extremely important initiative. “People of all ages need to take steps to stay connected with others for their health and mental well-being.  It’s not just a “nice to have’ this kind of project, it’s a critical need to have for the health and well-being of our nation,” she says. 

While there is no federal funding allocated to fund implementation of the advisory’s recommendations, a divided Congress might just be able to come together to address the personal and societal problems caused by loneliness and isolation. “This is an issue that affects both parties and we hope that it is viewed as a bi-partisan issue. Further, the healing power of social connection goes beyond individual health. The more communities, society, and leaders, can feel connected the more we can trust and rely upon one another and tackle many of the issues that we face as a nation,” adds Holt-Lunstad.

During this session of Congress, the Surgeon General’s advisory should be put placed on its policy agenda to hammer out new laws to enhance the nation’s social connections.  In concluding the advisory’s letter from the Surgeon General, Murthy warns:  “If we fail to do so, we will pay an ever-increasing price in the form of our individual and collective well-being. And we will continue to splinter and divide until we can no longer stand as a community or country.”

Tips on Connecting with Others

Below are recommendations to optimize and promote social engagement from the GCBH report. The recommendations are ordered so that the first ones might be appropriate for people who have very few social connections. This is followed by suggestions for those who are relatively socially active. The final recommendations are for those people who are already socially active. The GCBH also recommends that these people consider increasing the diversity or variety of their engagement. It is recommended that people should generally maintain a variety of the suggested types of engagements.

To promote meaningful social engagement:

1. Focus on the relationships or social activities you enjoy the most.

2. If you have no one around who can help you engage socially, turn to professionals who can assist. a. Examples: telephone hotlines, drop-in centers, a chat with a local religious leader, etc.

3. If you feel lonely, you can try to change this by making a new connection or by seeking different opportunities to engage with others.

4. If there are barriers to interacting with people (e.g., difficulty getting around, unsafe neighborhood), see if you can identify someone you could ask for help, and let someone assist you in making connections.

5. Try to keep a circle of friends, family or neighbors with whom you can exchange ideas, thoughts, concerns and practical matters, and who can also help or encourage you. It does not need to be a large group of people as long as those in it are important to you and you are important to them. Try to have at least one trustworthy and reliable confidante to communicate with routinely (e.g., weekly), someone you feel you can trust, and you can count on.

6. If you are married, this can benefit your cognitive health, but you should consider fostering other important relationships. Individuals who have never married or are divorced or widowed often have many other connections that provide support.

7. Try to speak every now and then (e.g., monthly) with relatives, friends and/or neighbors; communicate in person, or by phone, email or other means.

8. Help others, whether informally or through organizations or volunteer opportunities. For example, visit a lonely neighbor or friend, shop for/with them, or try cooking together.

9. Maintain social connections with people of different ages, including younger people. Keep in touch with grandchildren or volunteer to help people at a local school or community center. Think about the skills you have and that you use routinely that might be valuable to pass on to others. Offer to help teach a younger person skills you may already have, such as cooking, organizing an event, assembling furniture, saving for the future, investing in the stock market, etc.

10. Add a new relationship or social activity you didn’t try before. Place yourself in everyday contexts where you can meet and interact with others (e.g., stores or parks).

11. Be active and challenge yourself to try out organized clubs, courses, interest groups, political organizations, religious gatherings, or cooking classes.

12. If you are already socially active, diversify your activities. Consider joining or starting a group that doesn’t exist in your community and is centered around a common interest (e.g., a workout group).

Practical tips for those who have trouble engaging socially:

1. People can take small steps to connect with others. Share a smile a day with someone, show interest in someone by asking how they are, hold a door for someone, and practice a random act of kindness.

2. Reach out to neighbors or acquaintances whom you may not have spoken to in a long time: for example, call, send a card, email, or check social media.

3. Look at the list of additional resources that we provide in Appendix 1 and consider using them.
